After clicking image above, click make a copy. This will be your personalized copy.

Make a new copy each time you are generating an Action Form/Reference Check by using the current link.

Make sure all information is filled out by looking at the Action Type steps at the top of the page .

The small gray triangles in the cells are dropdowns for information, just click twice.

Double click in sections with the word "Date" and a calendar will pop up. Just click on the date.

Be sure to attach required documentation when email your HR representative.
