Sheet Tips

  • After clicking image above, click make a copy.  This will be your personalized copy.  

  • Make a new copy each time you are generating an Action Form/Reference Check by using the current link.

  • Make sure all information is filled out by looking at the Action Type steps at the top of the page .  

  • The small gray triangles in the cells are dropdowns for information, just click twice.  

  • Double click in sections with the word "Date" and a calendar will pop up.  Just click on the date.  

  • Be sure to attach  required documentation when email your HR representative.  

  • Please send all new hires to Susana Caraveo.  

  • Please print out in portrait mode and check no gridlines.
